Never reuse a VideoToolbox decompression session.

Never reuse a VideoToolbox decompression session.

Due to bugs in VTDecompressionSessionCanAcceptFormatDescription(),
remove the logic that reuses decompression sessions and always create a
new one. Tighten up the change detection to only trigger on keyframes.
