Change PPB_VideoWriter Open function to return stream_id as out param.

Minor tweak to PPB_VideoWriter Open method. The host should generate the
stream_id. This forced me to change the C++ wrapper to take a Var and I changed
the reader Open method for consistency.

TCCWO seemed better to me since the caller didn't have to keep the parameter
around itself. But I am new to this stuff.

Not sure what I was thinking. The WebSocket code should probably be updated to
also use TCCWO, which until recently wasn't working properly.
